All Cancellations:

All cancellations are subject to a $20 processing fee. Please consider rescheduling within the necessary timeframe to avoid charges.

Initial Intake Appointments:
Cancellations or rescheduling made less than 48 hours prior to the scheduled appointment time are subject to a $100 cancellation fee.

Follow-Up Appointments:
Cancellations or rescheduling made less than 48 hours prior to the scheduled appointment time are subject to a $50 cancellation fee.

No-Shows:
Failure to attend a scheduled appointment without prior notice (“no-show”) will be charged the full visit fee.

Cancellation and rescheduling fees are charged to the payment method on file.

Payment is collected in full at the time of booking unless otherwise specified.

Fees for completed visits are non-refundable.

Missed appointments and late cancellations are not eligible for refunds.

All intake and consent forms must be completed and submitted prior to the initial intake.

Completion and submission is the responsibility of the patient or caregiver.
Appointments may be rescheduled if required forms are incomplete at the time of the visit. Standard cancellation and rescheduling policies apply.

Accessing Required Forms:

1. Within 1–2 business days of scheduling initial intake, a separate secure Patient Portal (PHR) welcome email will be sent to the email used to schedule.

2. Follow the link to activate your portal account.

3. Once logged in, navigate to the “Questionnaires” section to complete required intake and consent forms.

For policy purposes, cancellation notice is based on the time the cancellation request is received.
